I’m Dr. Daniel Milligan, a licensed naturopathic physician providing primary care and gender-affirming care in Portland, Oregon, and by telehealth throughout Oregon and Washington. I completed my doctorate at the National University of Natural Medicine followed by a primary care residency, and I’ve been licensed since 2015.

My practice is built around something simple: unhurried visits with a physician who listens. I offer direct-pay care with transparent pricing, and new patients are typically seen within a week — no long waitlists, no insurance gatekeeping for visits (labs, imaging, and prescriptions still run through your insurance).

LGBTQ+ affirming services are core to my practice, not an add-on. I provide gender-affirming hormone therapy [using an informed-consent model] for transgender, nonbinary, and gender-diverse adults, including initiation, ongoing management, lab monitoring, and coordination of referrals. Beyond hormone care, I offer the full scope of primary care in an environment where you’ll never need to educate your doctor about who you are: routine and preventive care, thyroid and hormone health, ADHD care, mental health support, and medical weight management including GLP-1 therapy — always weight-inclusive and shame-free, at the patient’s direction.

Outside the clinic, I’m a proud gay man living in the Pacific Northwest with my husband and our two dogs, where you’ll find me in the garden, at the gym, building Lego sets, running our D&D campaign, or belting along to Broadway soundtracks.

I also serve as a preceptor for naturopathic medical students, because building the next generation of affirming providers matters to me. I practice in person at Prema Health in Southeast Portland on Mondays and Wednesdays, with telehealth available Monday through Thursday for patients anywhere in Oregon and Washington.
