The Lifeline is comprised of a national network of over 200 local crisis centers, combining custom local care and resources with national standards and best practices. Centers for Disease Control and Prevention Lesbian, gay, bisexual, and transgender have a variety of health needs. Find networks, services, and health clinics by state available to address those needs. Center of Excellence for Transgender Health The Center of Excellence for Transgender Health at UCSF provides clinical manuals and guides for health professions, including the “Guidelines for the Primary and Gender-Affirming Care of Transgender and Gender Nonbinary People” resource. They also offer free online learning modules, brochures, and fact sheets about various aspects of transgender health. Family Acceptance Project The Family Acceptance Project is a research, intervention, education and policy initiative to prevent health and mental health risks for LGBTQ children and youth, including suicide, homelessness, drug use, and HIV, in the context of their families, cultures, and faith communities. They use a research-based, culturally grounded approach to help ethnically, racially, and religiously diverse families learn to support their LGBTQ children. Gay and Lesbian Medical Association The Gay and Lesbian Medical Association (GLMA) provides a free cultural competence four-part webinar series about the healthcare of LGBT individuals.

Group for the Advancement of Psychiatry (GAP) GAP provides an LGBTQ mental health syllabus that contains free modules about working with LGBTQ patients.

Human Rights Campaign (HRC) Foundation Use this tool to find healthcare facilities near you that are evaluated in the Healthcare Equality Index. The HEI 2022 features more than 2200 healthcare facilities nationwide, including those that actively took the HEI 2022 survey as well as those that were researched by the HRC Foundation. LGBTQ+ ECHO The ECHO Center at the IUPUI Richard M. Fairbanks School of Public Health has the LGBTQ+ ECHO to train providers in evidenced-based practices for offering respectful, patient-centered, culturally competent healthcare to all patients. National Coalition for LGBTQ Health The National Coalition for LGBTQ Health is committed to improving the health and well-being of lesbian, gay, bisexual, and transgender individuals through federal and local advocacy, education, and research. The Coalition addresses the entire LGBTQ community, including individuals of every sexual orientation, gender, gender identity, race, ethnicity, and age regardless of disability, income, education, and geography. National Hispanic Medical Association HispanicHealth.info provides extensive, accessible resources to members of our community in both English and Spanish. NHMA cultivates credible sources to ensure that the information we provide is accurate, reliable, and relevant, especially in HIV/AIDS resources for patients and providers. National LGBTQIA+ Health Education Center The National LGBT Health Education Center provides free online education and teaching materials about LGBT health topics, including modules, webinars, CME/CEU courses, and grand rounds. World Professional Association for Transgender Health (WPATH) WPATH provides standards for transgender healthcare through the Standards of Care resource, a clinical guide for health professionals to assist transgender and gender nonconforming people with effective approaches to primary care, mental health, hormone therapy, surgery, and other health services.

Alabama LGBTQ+ Health Resources

AIDS Alabama AIDS Alabama focuses on housing, policy and advocacy, supportive services, HIV prevention and education, as well as free and confidential HIV testing. The organization is devoted to helping people with HIV/AIDS live healthy independent lives. They offer several support groups and mental health services, an example of one is their unique support group for the Latinx LGBTQ community called Somos Nosotros. They also have a Living Well Out Patient Center that offer programs for HIV-positive patients with emotional and mental health issues. Birmingham AIDS Outreach Birmingham AIDS Outreach is an organization that offers free and confidential tests for HIV, Chlamydia, Gonorrhea, Trichomoniasis (TRICH), Hepatitis C and Syphilis. They host several testing events throughout the year at various metro area locations including college campuses, health fairs, retail locations and bars while also offering regular on-site testing. They offer services through their PrEP Clinic located at the University of Alabama and have case managers who are licensed social workers to help people living with HIV set personal goals and obtain any assistance they may need. C.A.R.E. Services, Inc. C.A.R.E. Services exists to provide for the mental and physical well-being of sexual and gender minorities, and to work in partnership with other marginalized populations to achieve mutual goals of equity and equality. They offer LGBTQ+ youth support groups every Tuesday from 6-8pm in Huntsville, Athens, and Dothan, as well as a transgender peer support group in Huntsville. Anticipated future services to include counseling/therapy, adult peer support groups, and social gatherings. Equality Alabama Equality Alabama is a nonprofit advocacy organization seeking to advance equal rights for LGBT Alabamians through education, advocacy, and collaboration.  Five Horizons Health Services (FHHS) Five Horizons Health Services offers services to West Alabama’s LGBTQ community. Services include specialized medical treatment, HIV/Hepatitis C testing and treatment, STD/STI treatment and testing as well as prevention education. Special programs include SISTA (Sisters Informing Sisters on Topics about AIDS) an African-American women peer-led program, Probationer and Parolee HIV/HCV Testing Programs to provide HIV and HCV education to the Tuscaloosa County and a Latinx Outreach program. Magic City Wellness Center The Magic City Wellness Center is a comprehensive LGBTQ center located in Birmingham, AL. They provide culturally competent primary care services including PrEP, counseling, HIV and STD/STI testing, hormone replacement therapy, and support groups for youth and adults. Medical Advocacy and Outreach (MAO) Medical Advocacy and Outreach provides much needed health and wellness services benefitting people living with HIV/AIDS, Hepatitis C and other life-threatening illnesses. Multiple full-service clinic locations across central and southern Alabama offer services geared towards the LGBTQ community. Services include physical and mental health providers, HIV education and testing, food bank services and other support services. Shoals Diversity Center Shoals Diversity Center in Florence, AL offers limited free individual and couples counseling to those without insurance, as well as a sexual assault support group, an LGBTQ youth group (ages 17 to 24), and a gender expansive support group. Thrive Alabama Thrive Alabama empowers LGBTQ patients to create a healthy community by providing compassionate, accessible, affordable and comprehensive care. They have HIV clinics in Huntsville, Florence, and Albertville and provide primary care and telemedicine services. Other services offered include STI testing and treatment and HIV prevention via PrEP and safe sex education. University of Alabama- Birmingham LGBTQ Mental Health & Wellness Clinic The UAB LGBTQ Mental Health & Wellness Clinic serves gender non-conforming children and LGBTQ+ adults. They offer outpatient medical treatment, outpatient substance abuse treatment, individual psychotherapy, group therapy, and letters of referral for HRT. Services performed by medical physicians, clinical psychologists, and social workers. VA Medical Center: Lesbian, Gay, Bisexual & Transgender Program The Birmingham Veterans Affairs Medical Center offers a variety of programs to create a health care system that is supportive and responsive to the particular needs of LGBT Veterans and staff. Transgender patients and intersex individuals are provided all care included in VA’s medical benefits package including but not limited to: hormonal therapy, mental health care, preoperative evaluation, and medically necessary post-operative and long-term care following sex reassignment surgery.

Alaska LGBTQ+ Health Resources

Alaskan AIDS Assistance Association Alaskan AIDS Assistance Association (Four A’s) is an organization that seeks to improve care and prevention of HIV/AIDS in Alaska. They provide HIV testing, clean syringe exchange programs, and medical case management to HIV positive patients. Choosing Our Roots Choosing Our Roots is a queer-led and queer-focused nonprofit providing housing support and case management to queer, Alaskan youth and young adults experiencing homelessness. Choosing Our Roots provides housing support, case management, bridge housing, host homes, and referrals to additional services. Identity Identity is an organization in Anchorage that acts as an advocate for the LGBTQ adults and youth of Alaska. They provide community programming, support groups, advocacy, and an LGBTQ-friendly medical provider directory. Interior AIDS Association Interior AIDS Association seeks to provide care and prevention resources for HIV/AIDS and is headquartered in Fairbanks. They provide HIV and Hepatitis C testing, medication assisted treatment for opioid use disorders, and medical case management for people living with HIV. Planned Parenthood - Anchorage Offers various primary care and non-medical exam services. Other services include birth control, HIV testing, and hormone therapy. Providence Alaska Medical Center Provides primary care, OBGYN care, hormone therapy, behavioral health, social work, and support.

Arizona Trans Youth and Parent Organization (TYPO) Arizona TYPO is a youth and parent organization that seeks to support trans youth and their families in Arizona. They provide support groups, educational resources, and assistance in accessing trans-affirming healthcare. Chrysalis Chrysalis is a community program and crisis shelter that provides resources to those affected by domestic violence in the Phoenix area. They provide crisis shelter, transitional housing, and counseling services based on availability. Gender Identity Center A mental health therapy practice that provides individual therapy, couples/family therapy, and group therapy to clients who are transgender, non-binary, gender fluid, gender queer, LGBTQ+ or just exploring! They work with kids, teens, adults, and families. Gender Management Service - Phoenix Children’s Hospital The Gender Management Service at Phoenix Children’s Hospital is the only comprehensive gender health program at a children’s hospital in Arizona. They provide comprehensive gender care that begins with consultation for pubertal blocking medications, gender-affirming hormone therapy, and legal and school assistance through the transition process. HIV Services - Chicanos Por La Causa LUCES HIV Services through Chicanos Por La Causa’s LUCES program provides HIV testing, treatment, and referral resources to the Latino community of Phoenix. Identity Hormones and Healthcare A specialty clinic offering exceptional medical care for LGBTQI+ communities and its allies throughout the Phoenix area. They specialize in hormone replacement therapies and modalities. One N Ten One N Ten is a non-profit organization that provides a youth center and educational resources to LGBTQ youth ages 14-24 in Phoenix. They focus on providing a safe space, discussion groups, and healthy lifestyle education. Peacock Mental Health Provides private and meaningful mental health care for the LGBTQ2IA+ and neurodiverse community of Arizona. Their practitioners work closely with you to navigate your journey to better mental health. A treatment plan often includes therapy, medication management, and lifestyle strategies. Planned Parenthood Arizona - Gender Affirming Care Center Planned Parenthood Arizona now offers expanded services for transgender and non-binary patients, including gender affirming hormone care, which is available in our health centers as well as via telehealth. Pima County Clinics: HIV and STDs Pima County Clinics promote healthy sexual behaviors by providing comprehensive services to prevent the transmission of sexually transmitted diseases and HIV. Services include diagnosis and treatment of sexually transmitted diseases, HIV testing and counseling, partner identification and treatment and community outreach and education. Southern Arizona AIDS Foundation Southern Arizona AIDS Foundation seeks to provide testing and treatment resources for HIV/AIDS in Southern Arizona. They provide HIV testing and counseling resources in addition to referral and resources for accessing medications for treatment and prophylaxis. Southwest Center for Transgender Services Southwest Center for HIV/AIDS seeks to provide care and prevention resources for HIV. They provide walk-in HIV/STI testing, comprehensive primary care, transgender healthcare including hormone therapy, PrEP/PEP, and a full range of other health services to those living with or at risk for HIV. Spectrum Medical Care Center Since 1998, Spectrum Medical has been a leading provider of LGBTQ+ healthcare in the Phoenix region. Located on E. Monterey Way in downtown Phoenix, Spectrum Medical provides an array of care options, including primary care, preventative care, transgender care, HIV care, PrEP (pre-exposure prophylaxis), and PEP (post-exposure prophylaxis) for HIV prevention and STI testing and treatment. Stonewall Institute The Stonewall Institute is a substance abuse and behavioral health organization located in Phoenix. They provide LGBTQ-integrative substance abuse and mental health care to the state of Arizona. Terros Health Terros Health seeks to provide comprehensive health care to the LGBTQ community of Arizona. Their services include primary care, substance abuse treatment, crisis intervention, and free HIV and STD testing and resources to Maricopa County residents. Transgender Medical Services - El Rio Health Transgender Medical Services at El Rio Health provides a full range of primary and specialized care to the transgender community of Arizona in a gender-affirming environment. They provide full primary care for adults, children, and adolescents including hormone therapy as well as limited OBGYN and cancer screening at certain clinic locations. Trans Spectrum of Arizona Trans Spectrum of Arizona is a support organization for the transgender and gender-variant community of Arizona. They provide weekly support groups and educational resources to transgender individual and their families. Tutera Medical - Transgender/Gender Diverse Center They offer hormone therapy in various forms. They'v helped thousands of transgender & gender diverse people in the greater Phoenix area with HRT during and after transition. Valera Health Valera Health offers LGBTQIA+ support groups, welcoming people from all sexual orientations, gender identities, and expressions. VA Transgender Clinic Provides specialized health care for veterans who are transgender, transsexual, intersexed, gender variant, and questioning of gender identify, regardless of stage of change. Services include hormone therapy and access to physicians, pharmacist, psychologist, and others experienced in transgender care. Does not offer gender reassignment surgery; provides post-operative care. It also provides support groups for transgender veterans.

Hot Springs Pride Alliance Hot Springs LBGT Alliance seeks to maintain a sense of community and awareness for the LGBTQ community throughout the Garland County. They offer social and educational opportunities for the community and aim to create a supportive network for the local LGBTQ community and allies. Jeesi's House Jessi's House is a home for LGBTQ youth in the River Valley, giving them a safe space to rest and regroup, build a support network, and access resources to improve the trajectory of their futures. Lucie’s Place Lucie’s Place serves Arkansas’s young LGBTQ adults who are experiencing homelessness by providing them with safe living environments, job training, and counseling services. Northwest Arkansas Equality Northwest Arkansas Equality is an educational, advocacy, and service organization that offers HIV testing, peer support groups, and social & community outreach events. It is also hosts Arkansas’s largest pride celebration: the Northwest Arkansas Pride. River Valley Equality Center The River Valley Equality Center is a community organization committed to non-violent advocacy for the rights of all humans with an emphasis on the LGBTQ community. They offer counseling services, drug and alcohol treatment and various other resources including a list of local providers who are sympathetic to the trans community. VECTOR Health & Wellness VECTOR Health & Wellness is committed to providing safe, private and understanding care and support to the LGBTQ+ community. Alliance Health Project - UCSF Alliance Health Project is a program through UCSF that provides comprehensive mental health care to the LGBTQ population of San Francisco. They provide counseling, psychotherapy, substance abuse treatment, and other LGBTQ-affirmative services. Berkeley Free Clinic Berkeley Free Clinic is a non-profit health clinic that provides acute primary medical care and testing services on a free or low-cost basis. They provide HIV/STD testing and treatment, OBGYN screening and care, and trans/queer sexual health care in an inclusive, culturally-competent environment. Center of Excellence for Transgender Health - UCSF The Center of Excellence for Transgender Health partners with UCSF Medical Center and various provider organization to increase access to appropriate care and health resources for the transgender community. Through UCSF they provide comprehensive trans primary care, hormone therapy, and gender-affirming surgical procedures. They also formulate national treatment guidelines for transgender patients and supervise provider training in trans health. Chrysalis Community Counseling Services Chrysalis Community Counseling Services provides inclusive, affordable, feminist counseling services to families in Sonoma County. Their feminist model informs their inclusive treatment that serves diverse family types and individual orientations. Dimensions Queer Youth Clinic Dimensions Queer Youth Clinic provides low-cost health services to queer, transgender, and questioning youth ages 12-25. They provide HIV/STD screening and treatment, preventative care, immunizations, PAP smears, pelvic exams, and mental health care in addition to low-cost transgender health care. Gender Pathways Clinic - Kaiser Permanente The Gender Pathways Clinic at Kaiser Permanente’s San Francisco Medical Center provides appropriate transgender care to the transgender and gender-expansive community of San Francisco. They offer hormone therapy, mental health care, sexual and reproductive care and screenings, and surgical evaluation and procedures. Lyon-Martin Health Services Lyon-Martin Health Services is a community health organization dedicated to comprehensive, inclusive healthcare for heterosexual, bisexual, lesbian, and transgender women. They provide primary care, gynecologic care, HIV screening and specialist care, and transgender health services, including hormone therapy and surgical referral. Pacific Center for Human Growth Founded in 1973, Pacific Center for Human Growth is the oldest LGBTQIA+ center in the Bay Area, the third oldest in the nation, and operates the only sliding scale mental health clinic for LGBTQIA+ and QTBIPOC people and their families in Alameda County. To enhance the mental health and overall well-being of the LGBTQIA+ and QTBIPOC communities by providing culturally responsive therapy, peer to peer support groups, community outreach services, and facilitated workshops. Positive Images Positive Images is a grassroots nonprofit organization serving Sonoma County since 1990. QueerCare QueerCare is a trans-led community care service that offers free in-person post-op care to folks undergoing gender-affirming surgeries in SF. QueerCare hopes to make it easier for trans, GNC, and and LGBTQ people to pursue their procedures knowing they have a community-based support system that will be there for them throughout the process. In addition to help with basic post-op care, volunteers also provide transportation to and from medical appointments, meal prep and light house chores, pet and child care, and social support and companionship. San Francisco Community Health Center San Francisco Community Health Center (formerly API Wellness) is an LGBTQ and people of color healthcare organization that provides comprehensive, culturally-competent care to the Bay Area. They provide primary care, HIV/STD testing and treatment, PrEP/PEP, and a full range of transgender health services including hormone therapy through various providers. San Francisco City Clinic The San Francisco City Clinic provides free or low-cost sexual health care to the Bay Area. They provide services regardless of immigration and insurance status. San Mateo County Pride Center The Pride Center is the first ever LGBTQ+ community center in San Mateo County. Sacramento LGBT Community Center The Sacramento LGBT Community Center works to create a region where LGBTQ+ people thrive. They support the health and wellness of the most marginalized, advocate for equality and justice, and work to build a culturally rich LGBTQ+ community. Services include sexual, mental, and general health. Housing and community-building services are also available for seniors, adults, and youth. Gender Health Center Gender Health Center seeks to provide comprehensive counseling and therapy services to the Sacramento area. They accept all patients but specialize in meeting the counseling needs of anyone identifying as gender-diverse and transgender. Golden Rule Services Golden Rule Services is a culturally-competent organization that seeks to address health disparities among people of color in the Sacramento area, particularly in HIV prevention in the MSM community. They offer HIV/STD testing and assistance in accessing resources such as PrEP. Weave, Inc. Weave seeks to address issues of domestic and sexual violence. The Source LGBT+ Center Our mission is to provide supportive spaces within our communities for LGBTQ+ people to learn, grow, belong, and thrive. They offer HIV prevention, testing, and support services like PrEP. Additionally, The Source LGBT+ Center partners with Tulare County Suicide Prevention Taskforce and Heartworks, Inc to provide LGBTQ+ affirming care to people who lack access to mental health services and support groups.

Southern Region

Riverside County

Desert AIDS Project The Desert AIDS project offers services in high quality primary care, HIV-specialty care with access to their HIV Health Center, a dental clinic and a sexual health and wellness center. They accept Medicare and Medi-Cal and have contracts with many insurance companies and offer a Sliding Fee Scale Discount for uninsured low-income clients. They specialize in services for those living with, affected by, or at-risk for HV or AIDS in the Coachella Valley. LGBT Community Center of the Desert The LGBT Community Center of the Desert offers free and low cost counseling for the LGBT community through their Scott Hines Health Clinic. They also offer wellness classes such as yoga and tai chi along with rapid HIV testing and PrEP education. Along with those services, every Thursday evening the center hosts a pop-up grocery store that provides free groceries for local families and residents without fear of discrimination of race, color, sex, sexual orientation, gender identity/expression, age or religion. TruEvolution TruEvolution works with high-risk and underserved LGBTQ populations to defend the lives of LGBTQ people by centering healthy equity and racial justice to advance health outcomes in underserved communities. They offer direct health services such as HIV/Hep-C testing, condom access, case management, emergency supportive housing, pharmacy access and health coverage enrollment. Being Alive Being Alive is a nonprofit organization focusing on the mental health and wellness of people living with HIV and AIDS. In the Meantime Men In the Meantime Men’s Group is purposed to enrich, empower and extend the lives of intergenerational black men, respectful of sexual orientation, through social, educational, health and wellness programs and services. Services offered include yoga and meditation, condom distribution, HIV testing and STD screening linkage, case management and health education outreach. Keck Medicine of USC They provide quality health care to transgender, non-binary and gender-diverse individuals in a safe and supportive environment. Services include affirming primary care and chronic disease management, gender-affirming surgical care, gynecologic care, hormone therapy, mental health care, occupational therapy, physical therapy, sexual health, STI testing/treatment, HIV care, PrEP, PEP, telemedicine, urologic services, and voice therapy. Latino Equality Alliance/Alianza Latina Por La Igualidud The Latino Equality Alliance promotes equality, and justice for the Latino/a Lesbian, Gay, Bisexual, Transgender, Queer + community. They are located at Mi Centro LGBTQ Community Center in Boyle Heights. This non-profit organization focuses on family acceptance, LGBTQ equality and immigration reform. They offer a variety of programs to help build healthy and safe environments for the LGBTQ community including a College Access and Success Support and an LGBTQA youth council that offers college application support and tips on how to create more inclusive spaces at home. LGBTQ Center Long Beach The LGBTQ Center Long Beach serves the LGBTQ community and allies in the Long Beach area. Services include HIV testing and STI screening services. They also offer resources in accessing PrEP in Long Beach along with mental health counseling services. Los Angeles LGBT Center The Los Angeles LGBT Center provides full primary care for both women and men and is a licensed medical services provider through the Los Angeles Department of Health. Specialized services including HIV/STD testing and treatment, PrEP/PEP, and other culturally-competent health services are provided through clinics at multiple center locations. Their transgender health program offers hormone therapy, pre- and post-surgical care, and trans-appropriate sexual health and cancer screenings. OUTReach Center The OUTreach Center serves the LGBTQ community and allies living in the Antelope Valley. They offer health, social, educational and cultural services that benefit the entire lifespan of LGBTQ people. Their services include support groups for members of the community to come together and empower one another. Rainbow Community Center Since 1995, Rainbow Community Center has built community, equity, & well-being among Lesbian, Gay, Bisexual, Transgender, Queer, Questioning & Intersex (LGBTQIA+) people & allies. They offer community mental health services, homeless transitional youth support, maintain a food pantry, HIV education and prevention programs, and other initiatives for youth and adults. San Fernando Valley LGBT Community Center The San Fernando Valley LGBT Community Center is a coalition made up of organizations serving the LGBT population in San Fernando Valley, especially among the Latino population. Programs include youth and adult workshops for STD/Disease prevention, counseling services, an HIV Examination Referral Program and community events. San Gabriel Valley Gay and Lesbian Center The San Gabriel Valley Gay and Lesbian Center fosters a safer and more inclusive San Gabriel Valley for people of all sexual orientations and gender identities. They offer community education programs such as PRIDETalk where the community gathers to learn about issues that effect the SGV LGBTQ Community. They also offer the following peer support programs: Senior 50+, Youth 12-18, Transgender, Queer in the ‘burbs, Addiction Recovery and Grief and Loss. Satrang Satrang serves the South Asian LGBTQ community by promoting awareness, acceptance and empowerment through social, education and advocacy-related events. The offer events such as A Night of Mujra to celebrate the queering of gender identity with a South Asian flare. They also offer support groups for South Asians who identify as transgender or gender non-conforming to meet and share stories and experiences. SoCal Club - Men's Health Foundation SoCal Club is a community center for queer youth of color ages 12-29, at 8601 S Broadway in South LA. Along with support groups and wellness classes, they offer HIV/STI testing, access to PrEP and PEP, connection to HIV care and treatment, and LGBTQ+-affirmative therapy. St. John’s Well Child & Family Center St. John’s Well Child & Family Center is an independent health organization that serves patients of all ages through various clinics spread throughout Central and South Los Angeles and Compton. They provide HIV/STD testing and treatment, primary care, and transgender health services and counseling. UCLA Health UCLA Health is committed to inclusive, culturally-competent care to the LGBTQ community of Los Angeles and the state of California. They offer specialized programs including their Gender Health Program which provides comprehensive medical care and support to the trans and gender variant community. They also house the CARE Center which conducts research and provides quality care to those living with HIV/AIDS. Van Ness Recovery House The Van Ness Recovery House offers residential drug/alcohol treatment regardless of the ability to pay. Their mission is to meet the needs of the LBGTQ community for alcohol and drug addiction recovery. They offer a 4-6-month program for residents to provide structure and support to deal with issues that may have lead to drug/alcohol use/abuse. They also offer a psychologist and therapist to help with internalized homophobia, childhood sexual abuse, acceptance of HIV and both undiagnosed and diagnosed mental health. The Youth Center The Youth Center is a youth drop-in center that serves homeless and at-risk youth in Los Angeles. It offers housing, meals, showers, drop-in services, and HIV testing and counseling on-site.

Ventura County

New Beginnings Center  The Center is staffed by a multi-disciplinary team of practitioners that includes licensed Marriage Family Therapists, licensed Physical Therapists, licensed bodyworkers, dietitians, and MDs. The Center provides a wide variety of outpatient mental and integrative health services and specializes in somatic approaches to healing and health. They offer various LGBTQ+ support groups and therapy services. Diversity Collective Ventura County Diversity Collective is a community-based 501(c)3 nonprofit governed by a volunteer board of directors and volunteer advisory board. Their mission is to provide advocacy, visibility, safety, and wellness for the  LGBTQ+ community. They do this work via their community resource center, programs, and community-building events. Orange County

LGBTQ Center Orange County At the LGBTQ Center OC, all members and allies of the Lesbian, Gay, Bisexual, and Transgender community of Orange County join together in a network of support and unity. San Diego County

The Center The Center of San Diego is a safe space, community center, and resource center for the San Diego LGBTQ community. They provide weekly HIV and Hepatitis C testing as well as educational and treatment referral resources. They also house the Hillcrest Youth Center which provides community resources and educational programs to LGBTQ youth. Family Health Centers of San Diego Family Health Centers of San Diego provide comprehensive, inclusive primary care to the LGBTQ community. They offer full primary care, HIV screening and treatment, PrEP/PEP, cancer screenings, and transgender health services including medical case management and hormone therapy. Compass Compass, located in Downtown Lake Worth, is an organization that provides comprehensive HIV services, youth programming, social support/groups, and hosts various outreach events throughout the Palm Beach County community. Jasmyn Jasmyn, located in Jacksonville, supports LGBT youth ages 13-23 by offering programming, support services, STD/HIV testing and treatment. Jasmyn also provides educational and diversity training to parents, guardians, and the community. Metro Metro is a collection of centers in the Tampa Bay area that provide comprehensive HIV services and medical care, social activities, classes, support groups, counseling, health and fitness programs, youth programs, substance abuse programs, older adult programs, behavioral health services, and free HIV testing. Community AIDS Resource and Education Services (CARES) CARES is a provider for HIV services across 11 counties in Southwest Michigan. NAMI Rhode Island NAMI support groups are free, confidential, peer-led, and offer participants the opportunity to share their experiences and gain support from other attendees. Pride in Aging RI SAGE Rhode Island is the state’s chapter of the SAGE organization committed to supporting and connecting aging LGBT community members. Heartland Health Heartland Health Resource Center is a nonprofit organization committed to promoting overall wellness for people living in Eastern South Dakota with HIV/AIDS and at-risk individuals. Transformation Project The Transformation Project supports and empowers transgender youth, young adults, and their families while educating communities in South Dakota and the surrounding region about gender identity and expression. Black Transmen Black Transmen is a nonprofit organization that provides numerous programs about economics, anti-bullying, STIs, suicide and domestic violence prevention, as well as transgender health. They also provide resources to individuals undergoing transition. Coalition for Aging LGBT The Coalition for Aging LGBT provides support for older LGBT individuals through help with housing as well as health necessities. They provide access to home care and advocate for the self-sufficiency of these individuals. Legacy Community Health Legacy Community Health provides health services in pediatrics, primary care, OB/GYN, and behavioral health. Lesbian Health Initiative of Houston The Lesbian Health Initiative of Houston advocates on behalf of the LGBT community for better access to healthcare and encourages the wellbeing of LGBT individuals. The Montrose Center The Montrose Center offers HIV/AIDS counseling, group therapy, and life counseling. They provide affordable substance abuse treatment for the LGBTQ population as well as an anti-violence program for survivors of sexual assault and domestic violence. Borderland Rainbow Center The Borderland Rainbow Center provides services such as therapy and support groups, but also educational and cultural workshops, social space, and a food pantry. El Paso GLBT Community Center The El Paso GLBT Community Center offers activities and support groups that aim to address the needs of the GLBT and HIV-affected people living in and visiting the El Paso metropolitan area. El Paso HIV/Sexual Health Prevention Services Various HIV support programs in El Paso. One such service is the CLEAR Program which offers free one-on-one counseling to youth and adults. The evidence based program assists HIV positive or negative individuals to take charge of their lives, be proactive, set and meet goals, learn ways to problem solve, and address current stressors in their lives. Pride UTHSA PRIDE is a multidisciplinary student organization composed of current and future health providers at the UT Health San Antonio. They have their own clinic with offering for all of the San Antonio LGBTQ+ communities.

Utah LGBTQ+ Health Resources

Encircle Encircle is an LGBTQ+ family and youth resource center that provides programming and education designed to support the LGBTQ community in Utah. They provide counseling and support groups for LGBTQ youth, adults, and their families. LGBTQ-Affirmative Psychotherapists Guild of Utah The LGBTQ-Affirmative Psychotherapists Guild of Utah is a professional organization that provides education regarding LGBTQ mental health issues and provides a directory of LGBTQ-affirmative therapists. Planned Parenthood Association of Utah Planned Parenthood Association of Utah provides women’s health and sexual health resources to the state of Utah. Their various clinics provide pregnancy care, contraception, abortion care, HIV testing and counseling, and STD testing and treatment across the state. Rebirth OB/GYN Rebirth OB/GYN providers OB/GYN, primary, and preventive healthcare to all LGBTQIA folks and have helped thousands of transgender and gender nonconforming folks transition (i.e., aid in legal, mental, medical, surgical and social transitioning). They offer hormone therapy, hysterectomies, and orchiectomies as well as suboxone, PrEP, medical marijuana recommendations, and electrolysis services. Employees are culturally-diverse and embrace all cultures and communities. Transgender Health - University of Utah Health The Transgender Health program at University of Utah Health provides comprehensive care to the transgender and gender-variant community of Utah. They provide counseling, hormone therapy, and gender-affirming surgical and cosmetic procedures using providers from a broad range of specialties. Utah AIDS Foundation The Utah AIDS Foundation seeks to increase awareness and improve care of those living with and at risk for HIV/AIDS in the state of Utah. Community Health Centers of Burlington CHC’s LGBTQ+ and gender-affirming health services offer a Pride Clinic twice per week for all members of the LGBTQ+ community. These donations are redistributed as grants to various LGBTQ organizations in Virginia. Entre Hermanos Entre Hermanos specifically focuses on gay, lesbian, bisexual and transgendered individuals within the Latin community. The organization ties the two communities together by first and foremost providing information. Members strive to create an environment that foster leaders for the future and encourage all participants to actively partake in human-rights movements and political/social change. AHAT Homecare AHAT Homecare provides assistance to people living with HIV/AIDS and Hepatitis C in Washington State. Gender Alliance of the South Sound The Gender Alliance of the South Sound is a federal 501(c)(3) non-profit group dedicated to assisting the transgender community within the South Puget Sound area of Washington state. G.A.S.S. is open to all individuals who wish to learn more about their own unique inner self, to meet and mingle with others who are likewise on this voyage of discovery, and to get assistances with transgender-specific resources. Based in Charleston, they provide social and educational resources to the state’s LGBTQ community and allies. Morgantown Pride Morgantown Pride provides social and educational resources for the LGBTQ+ Community, Friends and Allies in Mon. County. Planned Parenthood South Atlantic Planned Parenthood South Atlantic provides gender affirming hormone therapy (at some locations) as well as STI/HIV services. Tri-State Gender Collaborative The mission of the Tristate Gender Collaborative is to provide support, education, resource-sharing, and relationship-building among community organizations, care providers, advocates, and community members from diverse disciplines and backgrounds. Their goal is to promote the health, wellbeing, and rights of all transgender, nonbinary, and other gender diverse communities in the region with a focus on equity and inclusion. West Virginia University LGBTQ+ Center The LGBTQ+ Center offers inclusive programming and outreach initiatives for WVU students, faculty, staff, and broader community members. They collaborate with and support the university’s larger social justice efforts by providing inclusive, academic, and curricular development education on all WVU campuses, by advocating for members of the community, and by building coalitions and services. Brady East STD Clinic The Brady East STD Clinic is a no-cost clinic that provides comprehensive STD screening and HIV/AIDS prevention treatment and counseling. It provides treatment and resources with special regard to gender identity and provides a safe space for the LGBTQ community. Diverse and Resilient Diverse and Resilient seeks to contribute to a future where LGBTQ people in the state of Wisconsin lives full, healthy lives with a specific goal of addressing LGBTQ health disparities. They oversee multiple programs that provide services such as condom distribution, HIV testing and counseling including PrEP clinic, and educational curricula on LGBTQ issues to local schools. FORGE FORGE provides a directory of LGBTQ and specifically trans-friendy providers within the state of Wisconsin across various specialties. They also sponsor numerous programs and support groups throughout the state that address issues of violence, trans aging, and more. Their website provides access to multiple publications that address similar issues and are actively engaged in transgender advocacy. Holton Street Clinic The Holton Street Clinic provides specialized healthcare to all populations with actual or perceived STD’s. Services include comprehensive STD screening, HPV and HSV treatment, and vaccinations including HPV and Hepatitis A & B. HIV and syphilis testing are offered at no cost. Out Professional Engagement Network (OPEN) Out Professional Engagement Network, or OPEN, is a Madison-based non-profit that is committed to building equitable and inclusive experiences, workplaces, events and opportunities within the LGBTQIA2S+ community. Planned Parenthood of Wisconsin Planned Parenthood of Wisconsin provides affordable sexual health and reproductive care for the state of Wisconsin. Services offered include STI testing and treatment, cancer screening, pregnancy testing and care options, abortion care, and various preventative services. The Center: 7 Rivers LGBTQ Connection The Center: 7 Rivers LGBTQ Connection offers a welcoming space for all people regardless of sexual orientation or gender identification. They provide a wide variety of support groups, organize LGBTQ+ friendly social events, and advocate for LGBTQ+ friendly policies and legislation. Teens Like Us Program Teens Like Us is a program through Briarpatch Youth Services that seeks to educate and support LGTBQ youth ages 13-18. They host support groups and offer counseling on diverse topics such as HIV prevention and self-esteem building. University of Wisconsin Trans Health Services University Health Services at the University of Wisconsin offers services for transgender and genderqueer students. A specific LGBTQ health ambassador and counselor is available for mental health concerns and all UHS providers are trained in dealing with LGBTQ students and provide referrals to an established list of specialists. University of Wisconsin Health Gender Services University of Wisconsin Health Gender Services seeks to help transgender, gender-expansive, and non-binary patient access quality, gender-affirming care. The directory of providers across specialties including primary care, pediatrics, behavioral health, and surgery allows patients access to comprehensive and multidisciplinary care. Vivent Health Vivent Health provides inclusive, holistic, and integrated health and wellness services to individuals affected by HIV & other STIs, LGBTQ communities, and those seeking a welcoming and affirming health and community center. They provide free, rapid HIV and other STI testing at multiple sites and offer primary care at an on-site clinic. Walker’s Point Community Clinic The Walker’s Point Community Clinic provides primary care, urgent care, and basic care to underserved populations in Milwaukee. Provides referral for patients in need of free meal programs, mental health counseling, and substance abuse services. They accept patients from all backgrounds.

Cheyanne Regional Medical Center Offering hormone therapy, gender-affirming surgeries, and mental health services. KnoWyo KnoWyo is run by the Wyoming Department of Health and provides education and resources for free, confidential HIV testing at multiple locations throughout the state. They also provide vouchers on their website that can be redeemed for free testing at many clinics throughout Wyoming. Laramie Reproductive Health Laramie Reproductive Health provides affordable and comprehensive sexual and reproductive health care to the Laramie community. They provide STI/HIV testing and treatment and accept vouchers from KnoWyo for low-cost testing. Wyoming Equality Helping create equity for the LGBTQ+ community through advocacy work, providing resources, and putting on events. They also over virtual support groups and care navigation support.
