The intake form asks your “birth sex” and nothing else. It doesn’t ask about your gender identity or ask for your pronouns. It doesn’t ask if you’ve had surgery or your sex changed. It just asks birth sex, that’s it. I don’t think that’s affirming at all. The HIPAA privacy form is a bit concerning as well since it just says you’re giving them permission to “use and disclose” your health information.
