I am Dr. Kevin Tan, a cosmetic and general dentist and the founder of Form & Function Dental Aesthetics in Las Vegas. My practice was created with the belief that dentistry should feel personal, respectful, and affirming. Every patient deserves to feel safe, seen, and cared for without judgment, especially patients in the LGBTQ+ community who may have had past experiences where healthcare felt rushed, dismissive, or uncomfortable.

As a provider, I focus on creating a modern, inclusive, and personalized dental experience where patients can feel confident sharing their goals, concerns, health history, and identity. I welcome LGBTQ+ patients, transgender and gender diverse patients, same sex couples, chosen families, and anyone seeking a dental home where they are treated with dignity and respect.

My clinical focus includes general dentistry, cosmetic dentistry, porcelain veneers, smile makeovers, TMJ and bite therapy, airway focused care, sleep and snoring screenings, and wellness oriented dental care. For many LGBTQ+ patients, dentistry can be deeply connected to confidence, identity, self expression, and quality of life. Whether a patient is coming in for routine preventive care, addressing dental anxiety, improving their smile, or preparing for an important personal or professional milestone, my goal is to provide care that feels collaborative, individualized, and empowering.

At Form and Function Dental Aesthetics, we take time to understand the whole person, not just their teeth. Our new patient experience is intentionally comprehensive, allowing space to discuss medical history, dental concerns, esthetic goals, airway and sleep concerns, jaw or TMJ symptoms, and any personal preferences that help make the appointment more comfortable.
