I am Kristin Evans, FNP-C, a board-certified family nurse practitioner, Precision Nutrition Level 1 certified coach, and founding provider of Groundwork Health LLC in Chattanooga, Tennessee. I am a cis lesbian woman and a member of the LGBTQIA+ community. I built Groundwork Health in part because I know what it means to want a provider who actually gets it, and because I watched too many patients navigate a healthcare system that was not built with them in mind.

I have been providing gender-affirming hormone therapy for several years in primary care under an informed consent model. This means no gatekeeping, no therapist letters, and no referral requirements to begin hormone therapy. I work with transgender women and transfeminine patients seeking feminizing hormone therapy, transgender men and transmasculine patients seeking masculinizing hormone therapy, and non-binary and gender-diverse patients with nuanced or partial transition goals. If your goals do not fit neatly into a category, that is fine. We will figure it out together.

In addition to hormone therapy, I offer PrEP prescribing and management, STI testing and treatment, and comprehensive primary care for LGBTQIA+ patients through Groundwork Health’s direct primary care membership model. Direct primary care removes insurance from the equation entirely. Members pay a flat monthly fee and receive unlimited visits, direct messaging with me, and care that is coordinated around their life. No copays, no surprise bills, no insurance company involved in care decisions.

I am currently working toward WPATH certification. I am committed to ongoing education in gender-affirming care and staying current with best practices in transgender healthcare.

Groundwork Health serves adult patients 18 and older. Primary care and hormone therapy are available via telehealth to patients anywhere in Tennessee. In-person care in the greater Chattanooga area begins in early 2027. A limited number of reduced-fee memberships are available for patients with financial hardship. DPC memberships launch August 1, 2026. Nutrition coaching is available now to clients anywhere in the world.

I built this practice for patients who are done settling for less and who want a provider where they never have to edit themselves to be seen.
